Nominations must be supported by the following documents:

1. Nomination Form - which includes a write-up by the Principal/VP/HOD on how the teacher:
   • exhibits spirit of innovation and enterprise in use of ICT for the benefit of the school community;
   • inspires, influences and supports other teachers in their use of ICT in teaching; and
   • uses ICT (state the range of technologies, including one Microsoft technology*) effectively to support engaged learning in day to day lessons.

2. A lesson idea/plan designed and implemented by the teacher, involving the use of ICT (including at least one Microsoft technology*).  The lesson should illustrate how the teacher harnesses the value-add of ICT and uses ICT innovatively to support teaching and learning.  A review of the lesson idea/plan must be included.
